Smart Choice MRI recognizes the importance of your privacy. We believe that protecting and securing private information is an issue of trust and we strive to manage and protect the personal information you provide to us in a fair and respectful manner. This privacy notice describes how we treat user data including Personal Information which is collected by us during your visit to our Site (both terms as defined below). “Smart Choice MRI”, “we” or “us” means Smart Choice MRI, LLC.

II.        Introduction

The Site may be used to communicate with us regarding an MRI, such as to schedule an MRI. You can provide your contact information on the Site, and we will call you back to get the additional information we need to schedule an MRI.


III.        Privacy of Health Information

Under certain circumstances, information you share with us through the Site may be subject to the requirements of the Health Insurance Portability and Accountability Act of 1996 and its implementing regulations (“HIPAA”) and/or state medical record privacy laws. IV.        Information We Collect and How We Use It

A.        Types of Information

When you use the Site, we collect two types of information from you:

  • Personal Information. We collect your Personal Information that you provide to us to enable us to contact you to schedule an MRI and to conduct any follow-up that may be needed.
  • Non-Personal Information. We also automatically collect information that you provide to us that does not constitute Personal Information. Non-Personal Information can identify, for example, the computing device used by you to access the Site. It may be collected, for example, through “cookies”, such as information related to how you use the Site. Non-Personal Information can also include anonymous or aggregated data.

We use your Personal Information to contact you in order to schedule an MRI and to conduct any follow-up that may be needed.

C.        Anonymous Data and Data aggregation

We may anonymize or aggregate Personal Information so that individuals are not identified or identifiable from it, in order to use it for additional purposes, for example, for statistical analysis such as analysis of trends. We may share anonymized or aggregated data with our business partners and other third parties.

D.        How We Use the Non-Personal Information We Collect

Log Files. We automatically record certain Non-Personal Information that your browser sends to us when you visit the Site. We store this information as log files. These log files may include usage data such as the content of your queries, and such things as internet protocol (IP) address, browser type, browser language, internet service provider (ISP), and location data. We use Non-Personal Information contained in the log files, as well as some potential Personal Information, to perform analytics, engage in remarketing and behavioral targeting, display content from external sources, enable or engage in location based interactions, and for other purposes. D.        Links to Other Sites

The Site contains links to other web sites that are not owned or controlled by us, including a link that enables health care providers to access a third party web site for certain purposes relating to your care. We are not responsible for the privacy practices of such web sites. We encourage you to be aware when you leave the Site and to read the privacy notices and/or statements of each and every other web site that collects your Personal Information. This privacy notice applies only to Personal Information and Non-Personal Information collected by or on the Site.

VI.        Security

Securing your Personal Information is important to us. We take physical, electronic and administrative steps to prevent the unauthorized release of or access to your Personal Information. For example:

  • When a user communicates with us on the Site, the transmissions are encrypted using Secure Socket Layer (SSL) protocol.


  • A user’s access to the Site is controlled by a username and a password.

We reserve the right to modify this privacy notice and our privacy policies at any time by posting a revised notice on the Site. We may also choose to notify you by other means, such as email. We encourage you to review this privacy notice frequently. You can tell by the “last updated” date indicated below when this privacy notice was most recently modified.

IX.        Users Located Outside the United States

Information provided to us from outside the United States will be stored in our systems located in the United States. If you are located outside the United States, you consent to this transfer and you acknowledge that this privacy notice and our privacy policies with respect to any use we make of your Personal Information for compliance with our own legal and regulatory obligations (including those mentioned above) are governed by applicable U.S. federal and state law, regulations and rules.
